Boarding Release Form

    1. All pets must have been spayed or neutered.
    2. All pets must be current on the following vaccinations: Canines: Bordetella, Rabies, DA2PPV, Leptospirosis and K9 Influenza vaccines. Felines: Rabies and FVRCP vaccines. (Proof of vacccinations is required).
    3. American Animal Hospital Association (AAHA) guidelines recommend that dogs intended to be housed in boarding kennels or day-care facilities should complete any required vaccination at least 2 weeks prior to allow time for an appropriate immune response to develop. In the instance that a vaccine is givin inside this 2 week period, or while boarding, the owner will not hold Dale Mabry Animal Hospital or its staff liable if their pet(s) becomes sick or infected because of failure to follow the above AAHA guideline.
    4. All pets must be current on heartworm, intestinal parasite, and flea prevention.
    5. A yearly negative fecal test is required.
    6. All pets must be free of external parasites (i.e ticks, fleas, etc.), or they will be treated at the owner’s expense.
    7. Dale Mabry Animal Hospital has the owner’s permission to treat life threatening condition or emergency without his/her prior consent.
    8. If a sedative/tranquilizer is necessary for treatment or handling, Dale Mabry Animal Hospital has the owner’s permission to administer such medication.
    9. If the doctors of Dale Mabry Animal Hospital deem a non-emergency treatment necessary for the well-being of the pet, while boarding with us, they will contact the owner by phone and/or email to explain the necessary treatment and to obtain their approval. If the client does not contact us within 48 hours, we will proceed with treating the pet.
    10. Kennel sharing by pets from the same household shall be by the owner’s request. Only under certain circumstances will this be allowed, with the medical/managerial staff making the final decision. The owner of the pets must disclose any issues that could lead to an incident – history of aggression or food guarding, intact animals (i.e. not spayed/neutered), etc. Should pets be allowed to share a kennel, the owner assumes all responsibility for injuries, damage, etc that may occur. The doctors and/or staff may separate pets into different kennels should the need arise.
    11. Pets may be picked up Monday through Friday 8AM through 5:15PM (After 3PM if a bath is requested). Saturday from 8AM until Noon.
